FillinEyeDoc.com provides an online platform to facilitate contact between employers and doctors of optometry. Employers voluntarily post work opportunities and doctors voluntarily submit personal and professional information when they apply to or book jobs that will be viewed by employers for the purpose of selecting a prospective job candidate. It is up to the discretion of the employer to hire the doctor, regardless of the services provided, and fees charged, by Fill in Eye Doc.

As an Employer, I understand the following:
Fill in Eye Doc is a service that connects me with optometrists that I may hire if I choose. I am responsible for hiring the doctor, and I have no obligation to hire a doctor that i find on FillinEyeDoc.com. I am responsible for entering the information that I select to be posted publicly on this website, and I will not hold Fill in Eye doc responsible for any consequences of the information that I submit in my job postings. I will compensate the doctor directly for the professional optometric services they render, should I choose to hire them, and payment for these services shall not be paid through FillinEyeDoc.com. Payment amount, method, or schedule, for theses services shall not be negotiated by Fill in Eye Doc, but shall be negotiated between myself and the doctor I choose to hire; however, I shall honor the payment information I submit on my job post as a minimum amount in the case of a payment dispute. The doctors that apply to, and book, my jobs are neither employees nor independent contractors of Fill in Eye Doc, and I agree that I will hold harmless Fill in Eye Doc against any liability and expense, including attorney’s fees, arising from claims of the doctor’s violation of any applicable statutes, regulations, or of any harm to my property, staff, or patients seen by the doctor at my office. This website, and any of its owners/operators, affiliates and third-party service providers, are not responsible or accountable for the actions of, or accuracy of information submitted by, job applicants (doctors) on this site; or for the conduct or professionalism of potential employers who hold accounts on this site. I agree that the information I include in my job postings is true and accurate to the best of my knowledge.

Fill in Jobs:
I understand it is free to post a job on Fillineyedoc.com and I agree to pay the service fee of $10 when a fill in doctor books my job. I understand I will be charged $10 for each work date booked. The service fee is charged by FIED for providing a connection between myself and the fill in doctor, regardless of whether or not I hire them. I may choose to allow doctors to submit “bids” for the pay amount offered in my job post. If a doctor submits a bid, I may choose to accept or decline their bid and I am expected to respond within 48 hours. By accepting a bid I am agreeing to doctor’s bid amount and understand the doctor will be booked for my job. Once a doctor books my job, I will have access to the contact information the doctor has entered on their profile and they will have access to mine. I understand I may cancel a booking or delete a job post at any time, and that in no circumstances will there be any refunds, even if the booking is cancelled by myself or the doctor for any reason. The doctor will have the opportunity to leave a review about their experience working for me as their employer, which will be visible on my job posts to other doctors.

Full Time / Part Time Jobs:
I understand it is free to post a job on Fillineyedoc.com and I agree to pay the service fee of $10 when a doctor applies to my job. I understand I will be charged $10 for each doctor that applies. The service fee is charged by FIED for providing a connection between myself and the fill in doctor, regardless of whether or not I hire them. Once a doctor applies my job, I will have access to the contact information the doctor has entered on their profile and they will have access to mine. I understand I may delete a job post at any time, and that in no circumstances will there be any refunds, even if I choose not to hire the doctor for any reason. The doctor will have the opportunity to leave a review about their experience working for me as their employer, which will be visible on my job posts to other doctors.

Doctors for Hire:
If I decide to contact a doctor from the “Doctors for Hire” ads, I agree to pay the $10 service fee to access the doctor’s contact information. This fee is charged for facilitating contact between myself and the doctor and does not obligate me to hire the doctor, nor does it guarantee that the doctor will be willing or available to work based on the work preferences on their profile. There will be no refunds if I choose not to hire the doctor for any reason, or if they choose not to work for me.

Fill in Eye Doc does not take responsibility for, nor make promises or guarantees about, the professional credentials, abilities, license number or status or other personal or professional details claimed on this website by employers or doctors. Fill in Eye Doc does not take responsibility for any legal or personal foul play, misconduct, malpractice, negligence, or any other professional practice violation or lawsuit that results from the employment of a doctor through FillinEyeDoc.com. It is the duty and responsibility of the employer to: verify the accuracy of any and all information submitted by the doctor on FillinEyeDoc.com, including professional credentials and education; negotiate employment details such as compensation, schedule, and other relevant job details; decide which doctor to employ.

I agree to the following: my office staff and I shall abide by all applicable federal and state regulations relating to health and safety. I will be responsible for all related taxes and/or forms in requirements of IRS as result of this payment. I agree that I will be responsible for all related taxes and/or forms in requirements of IRS as result of service fees paid to Fill in Eye Doc.

I hereby agree to release the service provider’s (Fill in Eye Doc) owners/operators, its affiliates, and third-party service providers, and each of their respective officers, employees and agents from any claims, demands or damages (actual and consequential) of every kind and nature, known and unknown, suspected and unsuspected, disclosed and undisclosed, arising out of or in any way connected with use of this site. This website, and any of its owners/operators, affiliates and third-party service providers, are not responsible or accountable for the accuracy of information submitted by job applicants on this site; or for the conduct or professionalism of employers who hold accounts on this site.

Fill in Eye Doc is a service that connects me with employers that may hire me if they choose, and that Fill in Eye Doc is not an employer of optometrists. I am responsible for choosing which jobs to book or apply for, and I have no obligation to work at a job I book through FillinEyeDoc.com. I understand that it is up to the discretion of the employer to hire me for the job, and it is possible that I may not be hired. I acknowledge that I am an Optometrist by definition of Doctor of Optometry according to the Consumer Affairs Department and the National Board Of Optometry. I agree that if I am hired, I will perform my profession within scope of the practice of Optometry and that I will abide by all applicable federal and state regulations relating to health and safety. I understand that I am neither an employee nor an independent contractor of FillinEyeDoc.com and that this is NOT an employment contract and under NO circumstances may it be construed as such.

I agree that the information I submit on my profile on FillinEyeDoc.com is true and accurate to the best of my knowledge, and I give permission to the administrators of FillinEyeDoc.com to view this information at their discrepancy. I understand that my profile will be delivered to the employer each time I apply to or book a work opportunity and that my contact information will be provided to the employer when I book a fill in job or apply to a part time or full time job. I give the employer permission to view my information and contact me regarding the said work opportunities.

Fill in Jobs:
By "booking" fill in jobs on FillinEyeDoc.com I understand and agree to the following: my account will be charged a fee of $5 per work date for facilitating contact between myself and the employer, however there is no guarantee I will be hired and the employer may cancel the booking at any time. By "booking" fill in jobs or “applying” to full time or part time jobs on FillinEyeDoc.com I understand and agree to the following: I will have access to the employer’s contact information and they will have access to mine. FIED cannot guarantee the contact information from the employer’s profile is accurate, as it is up to the employer to enter their contact information correctly. There will be no refunds for any reasons, even if the employer chooses not to hire me or cancels the booking. The employer will have the opportunity to leave a review of my optometric services after I work for them, and I understand this review will be visible to employers when I book or apply to jobs on FillinEyeDoc.com.

Full Time / Part Time Jobs:
I understand that when I apply to full time or part time jobs, I will be given access to the contact information of the employer, and I authorize Fill in Eye Doc to give the employer access to my contact information so that we may get in touch for the purpose of discussing the work opportunity.

I acknowledge and understand that I will be compensated directly by the employer for my professional optometric services, should they choose to hire me.

I understand that my “Doctor for Hire” ad will be visible to the public, and FIED employers may choose to access my contact information. I understand that the purpose of the ad is to establish contact between myself and the employer and does not guarantee I will be hired. I agree to keep my profile and contact information updated and accurate.

If I submit a bid for a fill in job, I understand and agree to the following: if I submit a bid for a payment amount that is different than what the employer offers and the employer accepts the my bid, it is expected that the employer agrees to honor my bid amount but it is not guaranteed and FIED cannot be held responsible for ensuring or enforcing this agreement. In case the employer does not pay the amount indicated in my bid, I do not hold FIED responsible. In NO circumstance will FIED intervene to resolve a pay dispute, or a dispute of any other type, between myself and the employer. Payment for my optometric services shall not be paid through FillinEyeDoc.com, and the payment amount, method, and time, for theses services shall not be negotiated by FIED, but shall be negotiated between myself and the employer that hires me. I am responsible for all related taxes and/or forms in requirements of IRS as result of this payment. I am also responsible for all related taxes and/or forms in requirements of IRS as result of service fees I pay to FIED.

I agree that I will hold harmless FIED against any liability and expense, including attorney’s fees, arising from claims of caused by me or employer’s or his/her employees. I understand that I will not be liable to the employer’s office(s) or it’s employees for any claim, cost or fees arising from the services provided by this agreement, unless any such claims, costs or fees are judged by the appropriate court to be due to willful misconduct or gross negligence on my part. I shall provide proof of insurance/certificate of liability to the employer or his/her employees upon request. I agree that I shall, at all times, abide by the safety rules, regulations and working conditions of the employer’s office(s) and maintain the highest standard of professionalism and according to the Health Information Practice Privacy Notice (HIPPA) of it’s patients.
